Disciple Maker Index Survey

This survey will provide you the opportunity to reflect on your own spiritual growth and provide feedback on the efforts of the parish to help you grow.

The survey should take you between 10-15 minutes to complete.  Please do not skip questions unless instructed to do.  Your responses are kept confidential.  Your parish and Catholic Leadership Institute will not have access to individual survey responses.
